COVID-19 Safety Protocols & Campus Visitor Agreement
These COVID-19 Safety Protocols are in place through August 8, 2021.
COVID-19 Symptom Screening: Visitors are required to self-screen for symptoms prior to visiting campus. Visitors should not attend a campus tour if they display any symptoms of COVID-19 (cough, shortness of breath or difficulty breathing, fever or chills, body aches, fatigue, headache, sore throat, congestion, nausea, vomiting or diarrhea, new loss of taste or smell).
If you test positive for COVID within five days of attending the campus tour, you are required to contact email@example.com
Facial Covering (Mask) Policy: Effective June 2: Fully vaccinated people will no longer be required to wear face coverings in most indoor spaces on the UW-Madison campus. Unvaccinated people should continue to wear face coverings indoors. Clinical and health care settings may continue to require masks regardless of vaccination status. Masks are not required in outdoor settings; however, those who are not vaccinated are encouraged to wear them when physical distancing is not possible. The chancellor's order on masking has been rescinded.
If you are seeking a reasonable accommodation to the face-covering requirement due to a documented medical condition, you will need to request the accommodation AND submit medical documentation at least three weeks in advance of your tour date.
To get started with this process, please email firstname.lastname@example.org. In this email, please indicate you are requesting a face mask accommodation, AND the first and last name of the person in your party requesting this accommodation.
Please DO NOT submit medical documentation in this initial email. Once we receive your request, we will provide you with instructions on how to submit medical documentation.
Hand Washing Protocol: Staff and visitors are expected to follow hand washing protocols.
Physical Distancing: Six feet of physical distancing should be maintained with those outside your household while on campus.
Tour Cancellation: We reserve the right to cancel the tour at any time for any reason without rescheduling. Failure to follow safety protocols may also result in the cancellation of the tour.
